Users fill out the quiz. After quiz is completed, users get an email with quiz info populated on a form. Conversion happens when user e-signs the form.

Qualifiers: The caller must be the legal guardian of the injured party AND The injured party was age 7 or older at the time of intake and age 17 or under AND The injured party became addicted to social media (three hours a day for a minimum of four weeks on any specific platform, not combined) AND The injured party suffered from and received medical treatment for one or more of the following: Eating disorder Body dysmorphia Severe depression Severe anxiety Attempted suicide Completed suicide
